Forms define the structure of your clinical data: which fields you collect for a patient, a consultation, or a custom form. Built-in forms already exist. You can adapt them and, on the right plan, create your own.
How to get here
In the left menu open Data, then Forms.
What you see
- Title Forms and Manage your medical forms.
- Create custom form.
- Table: Name, Plural name, Actions.
- Per row: Edit form, Preview, and delete for custom forms only.
Custom forms appear first. Preview opens in a side panel.
Create a custom form
This requires Pro or Premium. Otherwise you will see an upgrade prompt.
- Click Create custom form.
- Enter Name and Plural name.
- Click Create. The form editor opens.
If you hit the plan limit: You have reached the form limit.
The new form also appears under Manage so you can capture records.
Edit, preview, or delete
- Edit form: opens the section and field builder.
- Preview: shows how filling the form will look. It does not save.
- Delete: custom forms only. Built-in forms cannot be deleted.
To delete, type DELETE (or eliminar) and acknowledge that related records will be lost.
Empty or error
- Empty: No forms found. / Create or import a form to get started.
- Error: Unable to load forms. Please try again. Use Try again.
Tip
Start by tuning the forms you use every day (patient and consultation). Create a custom form only when you need a record type that does not exist yet.
